FCSC requires a 20% deposit to reserve the OR

Balance of surgery must be paid in full two weeks prior to surgery by CC, money order or cash (no personal checks)

Cash Discount is only available if total amount is paid by cash option

The following cancellation policy applies to patients that schedule a procedure with the FLORIDA COSMETIC SURGERY CENTER:

14 DAYS OR MORE PRIOR TO SURGERY:
Reimbursement minus fee as designated below will be given or credit applied to future surgical procedure under the following guidelines:
1. If paid by cash, full credit or refund will be given LESS $100 cancellation fee.
2. If paid by credit, full credit or refund LESS any merchant fees charged to FCSC (generally 4-5%) plus $100 cancellation fee or $200 whichever is GREATER.
3. If paid by check, full credit or refund will be given LESS $100 cancellation fee after check has cleared the bank.

7-13 DAYS PRIOR TO SURGERY:
Fifty (50) percent reimbursement will be given or credit applied to future surgical procedure if cancellation/rescheduling is made for medical reasons or severe hardship. This is also subject to the merchant fee stipulation (2) and check stipulation (3) as stated above.

LESS THAN 7 DAYS PRIOR TO SURGERY:
NO REFUND will be issued if cancellation is made LESS than 7 days before scheduled surgery.

However, if surgery is cancelled or delayed for a verified medical condition requiring surgical delay, credit may be applied towards future surgical procedure.

Please consider that FCSC has invested extensive time and labor to schedule and assemble a medical team for your procedure. These medical personnel make a commitment to you to provide the highest quality care. FCSC will make every effort to fill a cancelled surgical time slot, but since this is not always possible we have made these policies in an effort to provide a fair and balanced approach to the handling of surgical fees.
